I have received, read and understand the terms and conditions of the WRCOG HERO Program Contractor Participation Terms and Conditions. I also understand that the approval of my registration is contingent upon my signing of the Contractor Participation Agreement (last page of the WRCOG HERO Program Contractor Participation Terms and Conditions) and completing a W-9 form and returning them by email to [email protected], by fax to (858) 385-0379 or by mail to WRCOG HERO Program, 4080 Lemon St., 3rd Floor, MS 1032. Riverside, CA 92501-3609. I understand that in order to get reimbursement from the WRCOG HERO Program for work provided to property owners, the Program must have received my signed copy of these documents.
